Self-Assessment Test (DSM-5) – Borderline Personality Disorder

Self-Assessment Test (DSM-5)

Self-Assessment Test (DSM-5)

This test is an initial self-assessment based on DSM-5 criteria. It does not replace a professional diagnosis. Answer based on how you have felt or acted in recent years.

1. Do you have an intense fear of being abandoned and make desperate efforts to avoid real or imagined abandonment?
Fear of abandonment is a core symptom of BPD. People with BPD may feel panic or anger even during temporary separations or changes in plans.
2. Are your relationships intense and unstable, alternating between idealizing and devaluing the other person?
Alternating between seeing people as “perfect” (idealization) and “awful” (devaluation) is called “splitting” and is common in BPD.
3. Have you ever had moments where you lacked a clear sense of who you are (unstable identity)?
People with BPD may struggle to maintain a stable sense of self, frequently changing goals, values, careers, or social circles.
4. Have you ever acted impulsively in ways that could harm you (e.g., excessive spending, unprotected sex, substance abuse, reckless driving, binge eating)?
Impulsive and potentially self-destructive behaviors are common in BPD, especially during times of emotional stress.
5. Have you ever engaged in self-harming behaviors, gestures, or threats of death?
Self-harming behaviors are an important criterion for BPD diagnosis and often occur in response to feelings of rejection or abandonment.
6. Do you experience intense mood swings that can last from a few hours to a few days?
Emotional instability is a hallmark of BPD, with rapid shifts between joy, anger, anxiety, and sadness.
7. Do you often feel empty inside?
Chronic feelings of emptiness are common in BPD and may lead to attempts to fill this void with impulsive behaviors or intense relationships.
8. Do you have difficulty controlling anger (e.g., intense anger outbursts, physical fights, or verbal explosions)?
Difficulty regulating emotions, especially anger, is a common symptom of BPD and can lead to interpersonal conflicts.
9. During times of stress, have you experienced paranoid thoughts or dissociative symptoms (e.g., feeling disconnected from yourself or reality)?
Dissociative symptoms and transient paranoid ideation can occur in BPD, especially during stressful situations or perceived abandonment.
10. Do you have a pattern of frantic efforts to avoid feelings of rejection or loneliness (e.g., entering relationships quickly, ending relationships prematurely, or avoiding relationships due to fear of rejection)?
People with BPD often adopt extreme behaviors to avoid feelings of rejection or loneliness, oscillating between intense attachment and withdrawal as a form of self-protection.
